Documentation needed for each player:
  • Player Photo
    • Player must be looking straight at the camera
    • Photo should include the shoulders to the top of the player's head
    • Player must not have anything on their face (i.e. eye back) or anything on their head (i.e. hat, hoodie, etc.)
  • Birth Certificate (Copy is fine)

  • Pop Warner Medical Form and the PIAA Form (Section 6). Both forms must be signed in 2025

  • Pop Warner Participant Contract and Parental Consent Form Must be signed in 2025

  • Player's Last Report Card
    • Last Report Card of the 2024/2025 School Year
    • Pop Warner National Requires a GPA of 2.0/70%
    • Scholastic Eligibility Form is Required for those that do not meet the requirement.
What equipment should my player expect to receive?

  • Helmet and Shoulder Pads
  • Game Jersey
  • Game Pants
  • Practice Jersey

What equipment do I need to purchase for my player?

  • Cleats
  • Practice Pants
  • Mouthpiece
  • Water Bottle
